Incoming 6th Grade Required Immunization

 

All incoming 6th grade students are required by the Department of Health to have one dose of Tdap vaccine given no earlier than their 10th birthday and one dose of Meningococcal vaccine given no earlier than their 11th birthday to begin school in September. This includes students who will be virtually learning.

If your child does not turn 11 until after the school begins, please make an appointment and provide the nurse with documentation of the scheduled appointment. 

 

If you are uninsured or if your insurance does not cover physicals or immunizations please see document below for information from the Hamilton Township Department of Health.  They can provide this for free.
