Medication Forms and Action Plans

 

In order for a student to recieve a prescription or over-the counter medication from the nurse, a completed medication authorization form is required and signed by both the doctor and the parent/guardian. Additional forms may be required for certain health conditions such as asthma, food allergies, diabetes and seizures.  A student may only carry their own medication with a completed Self Carry form from their parent/guardian and approval from their doctor.

*Please note that all medications must be brought in to school in their original container, no loose pills or pills in bags will be accepted. Students may not carry pills with them, they need to be stored in the nurse's office.*
